Balance Training Combined with Home Age-Friendly Modifications Effectively Reduces Fall Injuries Among Older Adults

In recent years, population ageing has continued to deepen, and falls among older adults have become a critical challenge in global public health. According to data from the World Health Organization, fall-related injuries are the leading cause of injury-induced death and disability among people aged 65 and above. A single fall can easily lead to fractures and traumatic brain injuries. It may also cause older adults to fear walking and reduce physical activity, further accelerating muscle loss and frailty, thus creating a vicious cycle. Geriatric studies from multiple countries have confirmed that an integrated intervention programme combining balance training and home age-friendly modifications can significantly reduce the incidence of falls and related injuries among older adults.

Falls in older adults stem from two major risk dimensions. The first is intrinsic risk brought by declining physical functions. As people age, reduced muscle strength, impaired balance and slower reaction speeds, namely sarcopenia and frailty, all raise fall risks. The second lies in hidden hazards within home environments. Uneven floor levels, slippery bathrooms, toilets and shower areas without handrails, cluttered walkways and insufficient night lighting are common triggers of falls at home. Reliance solely on medication or home renovation cannot fully eliminate fall risks.

This integrated intervention model has been promoted by elderly care institutions across Europe, America and many Asian countries. In terms of physical intervention, professionals tailor simple balance training programmes for older adults, including low-intensity exercises such as in-situ weight shifting, single-leg standing and stepping drills, paired with moderate resistance training. These exercises strengthen lower limb muscle strength, improve proprioception and help older adults stabilise their bodies independently. No large equipment is required, and the training can be completed at home with good safety and accessibility.

Environmental modification focuses on risk assessment and optimisation in domestic settings. Common upgrades include installing anti-slip handrails and mats in bathrooms, removing threshold height differences, clearing clutter in walkways, fitting motion-activated night lights and adding bedside guardrails. Such age-friendly adjustments aim to eliminate home hazards. Even when older adults experience unsteady gait, these measures can lower the chance of falling or reduce the severity of injuries if a fall occurs.

Follow-up research on community elderly health programmes in multiple countries shows that groups receiving both balance training and home age-friendly modifications see a marked drop in fall-related injury rates compared with control groups without interventions. This approach aligns with the concepts of healthy ageing and ageing in place, helping older adults maintain independent living abilities and preserve mobility dignity in familiar surroundings.

Researchers remind that intervention plans need personalised design. Older adults differ in physical condition, chronic diseases and living environments. It is recommended to conduct fall risk assessments first. Rehabilitation therapists and age-friendly modification consultants can then jointly develop training schedules and renovation checklists to avoid blind training or excessive renovation.

Industry experts state that fall prevention for older adults should not only rely on medical treatment. Proactive comprehensive prevention is essential. Balance training improves older adults’ inherent ability to avoid falls, while home age-friendly modifications optimise external living environments. This dual collaborative model is emerging as an important direction in global geriatric health management, supporting more older adults to enjoy safe, high-quality ageing at home.
